Description:

Provides health care to registered patients. Services include primary care for all ages, women's health care, physical examinations for employment and school, pediatric, geriatric, immunizations, health education and nutrition counseling, social services, on-site pharmacy, lab, X-ray (including mammography), and a range of medical specialty services. Provides integrated behavioral health care, including psychiatric evaluation and medication management.

Hours:

Mon, Tue, Thu, Fri 8am-5pm; 1st, 3rd Wed 9am-5pm, 2nd, 4th, 5th Wed 8am-5pm

Program Fees:

Sliding scale for income eligible; accepts AHCCCS, Medicare, and private insurance

Languages:

English, Spanish; other languages through phone interpreter service
