Description:
Provides case management to patients who have life threatening or debilitating diseases and are experiencing healthcare or financial issues that are impacting their care. Includes information about health insurance coverage, the Americans with Disabilities Act (ADA), and the Family and Medical Leave Act (FMLA). Offers assistance to apply for Social Security Disability Insurance (SSDI), to negotiate discounts, and set up payment plans. Does not offer medical advice or coordinate direct care.
