Provides case management services for seniors and adults with physical disabilities. Client services include comprehensive intake, referral services, development and implementation of individual care plans, home delivered meals, personal care, adult day respite, and housekeeping.

Mon 9am-2pm, Tue-Fri 8am-5pm
