Description:
Provides primary medical and behavioral health care for children and adults. Services include physical exams, psychiatric assessment, medication monitoring, care coordination, peer and family support, and vocational and employment services.

Mon-Fri 8am-5pm
Program Fees:
Fixed; accepts AHCCCS, Medicare, and some private insurance

English; other languages by arrangement
