Description:
Provides post-hospital transitional care for seniors discharged from Santa Cruz Regional Hospital. Services include prescription pick-up and grocery shopping. Also includes health education, care coordination and advocacy, medication education, explanation of discharge instructions, and follow up by a nurse for 30 days.

Mon-Fri 8:30am-5pm
