Residents of Santa Cruz County, with income below 250% of the Federal Poverty Guidelines
Intake Process:
Call for appointment. Must bring photo ID, rent or utility bill, proof of income from last 30 days.
Description:
Provides a medical discount program for residents on low income. Medical care is provided by health care professionals who have agreed to accept sliding scale fees which are based on family size and income.
Hours:
Mon, Wed, Thu, Fri 8am-4:30pm; Tue 9am-4:30pm
