Eligibility:
Children and adults with developmental disabilities as defined by state statutes (autism, cerebral palsy, down syndrome, intellectual disability, epilepsy); must be an Arizona resident
Intake Process:
Contact DDD Support Coordinator to complete application, or call the Customer Service Center.
Description:
Provides free identification bracelet or shoe tag to DDD Members, regardless of eligibility type, to help First Responders assist the member in an emergency situation.

Mon-Fri 8am-5pm
