People age 60 and older, and adults with disabilities ages 18-59. Does not accept those on ALTCS.

Description:
Provides intake and case management for seniors and adults with disabilities so they can remain in their own homes. Includes assessment of needs, development of an individualized care plan, and coordination of in-home support services.
Hours:
Mon-Fri 8:30am-5pm
Program Fees:
Participants pay a Share of Cost, if possible
