Intake Process:
Visit website for application form #96-0104. Must be signed by an Arizona physician, registered nurse, nurse practitioner, audiologist, or hospital administrator. Visit any MVD office or authorized third party office, or write to request disability plates and placards.
Description:
Provides special license plates which allows people with physical disabilities to park in reserved parking spaces marked with the International Symbol of Access. The plate is assigned to one vehicle in which the applicant must be an owner on the title. Also available is a free placard that can be displayed in a vehicle in which the person is a passenger, designating disability parking privileges for the vehicle. The placard is assigned to the applicant for use in any vehicle, and may not be transferred to, or used by, another person.
