Single adults with a SMI diagnosis who are AHCCCS eligible with Mercy Maricopa Integrated Care; must have referral from case manager or a member of clinical team

Description:
Offers a recovery supported permanent supported housing program for adults age 18 and older in the care of Mercy Care who have been diagnosed with a serious mental illness and who are AHCCCS eligible. Provides wrap around peer support services in the community to help locate and maintain housing.
Hours:
Mon-Fri 8am-5pm; 24-hours on-call
