Eligibility:
Must be US citizen or legal resident who has not had an eye exam in 12 months or more, and has increased risk for glaucoma due to family history, race, or age

Description:
Promotes early detection and treatment of glaucoma and raises awareness of glaucoma risk factors for people in need. Provides free glaucoma educational materials and facilitates access to a glaucoma exam. Does not provide eyeglasses, prescription drugs, hospital services, or pay the fees of other medical professionals.

Mon-Fri 8am-noon Pacific Time; 24-hours website
Program Fees:
Vary; insurance is billed if available, consumer pays co-pay; no charge to uninsured people
