Eligibility:
Children from birth to 18 years of age. Must be covered by AHCCCS, be uninsured, or have insurance that does not cover immunizations, or be native American or an Alaskan Native.
Intake Process:
Call, visit clinic; bring all immunization records
Description:
Provides free immunization services monthly to eligible children from birth to age 18 in the City of Mesa inside Superstition Spring Center.
Hours:
5pm-7pm the second Wednesday of each month through December 2024.
