People age 14 and older who have disabilities

Description:
Provides assistance to help people with disabilities move out of nursing facilities, institutional settings, and other protected environments. The goals is to live independently in the community. Also provides assistance to help youth with disabilities to transition from high school to work, higher education, and/or living in their own homes.
Hours:
Mon-Fri 8am-5pm
