Child and adults who have been diagnosed with Serious Mental Illness (SMI) or General Mental Health/Substance Abuse

Description:

Provides primary medical and behavioral health care for children and adults. Services include preventative health care, nutritional counseling, pharmacy consultations, wellness programs, and chronic disease management. Also includes psychiatric assessment, medication monitoring, crisis intervention, care coordination, addiction counseling, peer and family support, and vocational and employment services, and housing assistance. Also offers individual, family, and group counseling.

Hours:

Mon-Fri 8am-5pm

Program Fees:

Fixed; accepts AHCCCS, Medicare, and some private insurance

Languages:

English; other languages by arrangement
