Children and adults, with an emphasis on those who are homeless or have behavioral health issues

Call for appointment; same day appointments may be available

Description:

Provides health care for people who are homeless and those with serious mental illnesses. Services include primary care, medical specialty services, pharmacy, lab, X-ray, health education, and nutrition counseling. Provides integrated behavioral health care, including psychiatric evaluation and medication management. Also provides gender-confirming hormone therapy for adults and referral to specialists. Does not provide immunizations at this location. Offers assistance with AHCCCS applications.

Hours:

Mon-Fri 7am-8pm health center; Mon-Fri 8am-8pm pharmacy

Program Fees:

Sliding scale for income eligible; accepts AHCCCS, Medicare, and private insurance

Languages:

English, Spanish; other languages through phone interpreter service
