Eligibility:

Adults with general mental health and/or substance use or adults with serious mental illness (SMI) designation. Those who are AHCCCS eligible, Title XIX and Non-Title XIX SMI adults enrolled in the Maricopa County RBHA or United HealthCare DDD services

Description:

Provides integrated primary and behavioral health care for adults diagnosed as SMI (Serious Mentally Ill). Provides outpatient mental health and substance abuse counseling services for SMI and GMH/SU (general mental health/substance use) adults. Services include case management, group and individual counseling, psychiatric evaluation, psychiatric medication management, and psychosocial rehabilitation. Therapeutic groups address substance abuse, general mental health, non-SMI co-occurring, anger management, women's intensive substance abuse day treatment, and basic dialectical behavior therapy (DBT) skills.

Mon-Fri 7:30am-5:30pm

Sliding scale; accepts AHCCCS and Medicare
