Description:
Provides street outreach and housing assessments to people who are homeless. Based on the assessment, those who are the most in need are placed on the community-wide priority list. Local agencies use the list to get clients. Includes screening for additional services. Mobile program for coordinated entry into housing. Offers water, hygiene kits, case management, and assistance with IDs.
