Children and adults in households with income at or below 200% of Federal Poverty Level Guidelines. Must have no coverage through private insurance and not received care through a VSP program during the last 12 months.
Intake Process:
Ages 19 and younger need to contact school nurse. People age 20 and older need to call or visit website.
Description:
Provides gift certificates for eye exams and new prescription glasses for children and adults in need. Gift certificates are distributed through local VSP network doctors and through school nurses.
