Eligibility:
Adults age 62 and older; homeowners with disabilities who are on low income; must live in Buckeye, Cave Creek, Gila Bend, Tonopah, Wickenburg, Wittman, Pinal and Gila County. Residents in Tempe and Peoria may also qualify.

Description:
Provides home modifications to increase accessibility and safety for people with physical disabilities. Services may include building ramps, widening doorways, installing grab bars, relocating electrical switches to wheelchair height or modifications to improve accessibility and safety. Mercy Care Arizona Long Term Care System members need a referral for home modifications from their case manager.
