Description:

Provides primary medical care services including prenatal services, well child, women, and men check ups, immunizations and flu shots, family planning, free pregnancy testing, pap smears, contraceptive counseling, chronic care management, diabetic management, procedures such as simple lacerations, wart removal, onsite laboratory services such as rapid strep testing, acute and chronic disease management, acute illness management, school and sports physicals, prenatal care, and other services. Provides free transportation for medical appointments for established patients only (within a 15 mile radius).

Hours:

Mon-Fri 8am-7pm; same day appointments may be available

Program Fees:

Sliding fee scale available. Accepts private insurance, RMA, and AHCCCS

Languages:

English, Spanish; other languages through interpreter service
