Description:

Provides primary medical care services including well child, women, and men check ups; immunizations; flu shots; family planning; free pregnancy testing; pap smears; contraceptive counseling; diabetic management; procedures such as simple laceration care and wart removal; onsite laboratory services such as rapid strep testing; acute and chronic disease management; school and sports physicals; prenatal care; and other services. Provides referrals to outside resources. Provides free transportation for established patients for appointments at NHW Community Health Center within a 15 mile radius of the clinic.

Mon-Fri 8am-7pm

Program Fees:

Accepts AHCCCS, RMA, and some private insurance. Sliding fee scale available.
