Description:

Provides primary medical care services. Includes prenatal services, well child, women and men check ups, COVID tests and vaccines, childhood and adult immunizations, flu shots, family planning, chronic care management, diabetic management, procedures such as simple lacerations, wart removal, onsite laboratory services such as rapid strep testing, acute and chronic disease management, acute illness management, school and sports physicals, prenatal care, and other services. Provides free transportation for medical appointments for established patients only within a 10 mile radius of the clinic.

Mon-Fri 8am-7pm

Program Fees:

Accepts AHCCCS, RMA, and some private insurance. Sliding fee scale available.
