Call or visit website for application and medical clearance form. Return them with a copy of the audiogram test results less than 6 months old.

Provides hearing aids and fitting services to children and adults who have no other resources. Services provided at local centers.

Mon-Fri 8am-5pm

None for children age 18 and younger, $200 nonrefundable application fee for adults
