Description:

Provides health care assessment and case management to Native Americans. Includes specialized care in substance abuse, diabetes, HIV/AIDS, tobacco prevention and cessation, health promotion and immunization monitoring. Also includes talking circles, sweat lodges, and Acudetox. Provides transportation to San Xavier Indian Health Clinic, and medication delivery.
