Description:

Provides primary health care for all ages. Services include women's health care, physical examinations for employment and school, pediatric, geriatric, childhood and adult immunizations, health education and nutrition counseling, social services, on-site pharmacy, lab, X-ray (including mammography), and a range of medical specialty services. Provides integrated behavioral health care, including psychiatric evaluation and medication management. Also provides gender-confirming hormone therapy for youth and adults, and referral to specialists. Offers a children's car seat loaner program.

Hours:

Mon, Tue, Thu, Fri 7am-5pm; 1st, 3rd Wed 9am-5pm; 2nd, 4th, 5th Wed 7am-5pm; Sat 8am-4pm

Program Fees:

Sliding scale for income eligible; accepts AHCCCS, Medicare, and private insurance

Languages:

English, Spanish; other languages through phone interpreter service
