Eligibility:
Individuals at risk of homelessness, currently experiencing homelessness, and or diagnosed with SMI or general mental illness. Re-entry program is for men and women being released from incarceration and/or have justice involvement who would experience homelessness after separation.

Description:
Provides rapid rehousing services, permanent supportive housing, and help locating affordable housing. Also provides men and women's community re-entry program.
