Description:

Family practice clinic provides well-woman exams, well-baby checkups, childhood and adult immunizations, and referrals to necessary specialists. Offers radiation exposure screening, and specialty clinic for HIV care and case management. Also provides comprehensive dental services for children and adults. Includes teeth cleaning, topical fluoride applications, fillings, treatment of periodontal disease, and extractions. Offers health benefits enrollment assistance.

Hours:

Mon-Fri 7am-6pm family practice and virtual appointments; Mon-Fri 8am-5:30pm dental

Sliding scale for income eligible; accepts Medicare, AHCCCS, and most private insurance
