Call for same-day drop-in and telehealth intake times
Description:
Provides integrated primary care and behavioral health care for adults living with mental illness, substance use disorders, and trauma. Services include physical exams, treatment for chronic conditions such as diabetes and high blood pressure, and immunizations. Also includes case management, counseling, psychiatric assessment, and medication management.
Hours:
Mon-Fri 8am-5pm
Program Fees:
Sliding scale for income eligible; accepts AHCCCS, Medicare, and private insurance
Languages:
English, Spanish; other languages by arrangement
