Alternative Medicine Referrals

Programs that maintain lists of alternative medicine practitioners who offer health care and treatment procedures that fall outside the mainstream of conventional medical practice and link individuals who are in need of care with qualified professionals.

Dental Care Referrals

Programs that link people who are in need of care which involves their mouth, teeth, gums or associated structures with dentists, orthodontists, periodontists or other members of the dental profession.

Dietitian/Nutritionist Referrals

Programs that maintain lists of dietitians and nutritionists, and link individuals with qualified practitioners who can help them develop healthy eating habits and change their diet, if necessary, to manage their weight, reduce their cholesterol levels, lower their blood pressure, deal with diabetes or prevent and/or treat other health concerns.

Eye Care Referrals

Programs that link people who are in need of vision care or medical services relating to eye injuries or diseases with practitioners who are qualified in the desired area of specialization.

Hospice Care Referrals

Programs that link people who are terminally ill with programs that provide supportive services during the final stages of their illness. Hospice care may be provided at home, in a freestanding hospice facility, a hospice unit of a hospital or in long-term care facilities such as nursing homes. Services may include medical care, pain and symptom management, home nurse visitation, case management, emotional and spiritual support, and bereavement services for the patient and members of the family.

Nursing Facility Referrals

Programs that link individuals who are in need of intermediate, secured or skilled nursing care with facilities that are appropriate to their needs and preferences.

Nursing Registries

Programs that maintain lists of nursing personnel including registered nurses, licensed vocational nurses/licensed practical nurses and certified nursing assistants, and link people who are in need of nursing care or facilities seeking personnel with one or more individuals who have the desired combination of qualifications and experience.

Physician Referrals

Programs that link people who are in need of primary or specialized medical care with qualified medical practitioners.

Rehabilitation/Habilitative Medicine Referrals

Programs that link people who have temporary or permanent disabilities with physical therapists, occupational therapists, speech and language therapists, respiratory therapists and other qualified rehabilitation professionals who can improve or restore the individual’s functioning, independence and overall quality of life.
